Online Form

Notes on the use of the Online Form

Note: You can assert data protection claims yourself against the data processing agency. Many questions and problems relating to data protection law can often be clarified quickly and easily in this way. If your concern has not been dealt with within a reasonable time or has not been dealt with satisfactorily, you can contact us.

The fastest way to process your request is to use the online form. Of course, you can also send us your request in a form of your choice by mail (contact see below).

Please note that the fields marked with an asterisk (*) (mandatory field) in this form are required fields that are necessary for us to process your request.

If you wish to send your request anonymously, you can enter “Anonymous” in the name field, for example. Please note that in this case we will not be able to send you an acknowledgement of receipt or inform you of the status and results of the procedure.

The more precise the information you provide on the facts of the case, the easier and faster it will be for us to process your request. If you have documents (e-mails, letters, photos or screenshots) relating to the matter, please send us the relevant documents.

We would like to point out that by submitting the online form, you agree that we may process your personal data automatically for the purpose of handling your request.

We will then consider your submission as a complaint within the meaning of Section 26 DSGJZ if you can demonstrate that the body about which you are complaining is processing personal data relating to you in such a way that your rights are being violated. If you do not comprehensibly demonstrate a violation of your own rights, we will consider your submission as a request for control.
